- 1). Visit NYDoctorProfile.com. This website contains the official records mandated by the New York Patient Health Information and Quality Improvement Act of 2000. The site includes general information about a doctor's license and education, along with details of any legal actions taken against your doctor.
- 2). Click "Search" and enter a whole or partial name to look up your doctor's profile. Use "Advanced Search" to search using other criteria, such as geography, hospital affiliation or medical specialty.
- 3). Review the doctor's profile. Click on any of the tabs to choose information on "Education," "Practice," "Legal Actions" or "Professional Activities." There is also an optional "Statement" tab where a doctor can provide his own commentary.
- 4). Click the "Office of Professional Medical Conduct (OPMC)" link under "Licensee Actions" to access details of any actions taken by the state licensing board against the doctor whose record you are reviewing.
