A 49-year-old man diagnosed 4 years ago with chronic hepatitis C presented to the emergency department for right upper quadrant pain and severe weight loss of 2 months' duration. He also reported black stools during the past 3 days.

Biochemical changes included low hemoglobin level (7.2 g/dL), increased aminotransferase levels (aspartate aminotransferase = 176 IU/L; alanine aminotransferase = 107 IU/L), and markedly increased gamma glutamyl transpeptidase (557 IU/L). The patient was anti-hepatitis C virus positive, hepatitis B surface antigen negative, and hepatitis B e antigen negative. Serum alpha-protein was markedly increased (600 ng/mL). Rectal examination disclosed melena.
