How to Order Birth Certificates From Oregon

    • 1). Download the "Birth Record Order Form" from Oregon.gov/DHS. Fill out the form, including the full name on the birth record, date of birth, place of birth, father's full name, mother's full maiden name, your name, address and telephone number. If ordering for someone other than yourself, set out your relationship to the named person and attach copies of the legal documents that make you eligible to order the birth certificate.

    • 2). Send the form (and any necessary supporting documents) with a check or money order for $20 for the first copy (and $15 for any additional copies) to the address listed on the form. These prices are as of March 2010 and are subject to change.

    • 3). To order a birth certificate online, go to VitalChek.com. The fee for orders made online is $32.50, as of March 2010, and can be paid by credit card.

    • 4). Order by fax by faxing the form and any necessary supporting documents to (503) 234-841. Include your credit card number and expiration date. Alternately, order by telephone with a credit card by calling (971) 673-1190 between 8:00 a.m. and 4:30 p.m. weekdays. Be prepared to provide all required information. The fee for ordering by fax or by phone is $32.50, as of March 2010.

    • 5). Order in person at the Vital Records Office, located at 800 NE Oregon Street, Room 205, in Portland. According to the Oregon Center for Health Statistics, the office is open weekdays from 8:00 a.m. to 4:50 p.m. Make your order before 4:30 p.m., and on average, you will need to wait in line for 20-30 minutes, (and longer on some days) so you should take that into account and arrive sufficiently early. The fee is $20 for the first copy and $15 for each additional copy, as of March 2010. Pay by cash, credit/debit card, check or money order. Provide photo identification and any documents necessary to prove that you are eligible to order the birth certificate.
