Carefully read the affidavit if the affidavit is submitted to you. Otherwise, write or type the affidavit. Include only factual information relevant to the case. Revise the affidavit after you have completed it and correct any mistakes. - 2

Visit a bank or court clerk and ask a notary public to officiate the affidavit. Take the oath the notary presents to you, repeating all information as commanded. Read the affidavit one more time, and sign it in the presence of the notary. - 3

Ask the notary to sign and seal the affidavit after you have signed it to make the document official. Make a copy for your records, and submit the original to the court from which it was issued.
