How to Fill Out a Checking Withdrawal Slip

    • 1). Place the checking withdrawal slip on a flat surface. Have your checking account number handy.

    • 2). Print today's date on the line next to the word "Date." This is the official date of the transaction.

    • 3). Write out in words the amount you wish to withdraw from your checking account on the line next to the word "Amount." For example, if you are withdrawing $350.46, write "Three-hundred fifty and 46/100 dollars."

    • 4). Insert the numerical amount you wish to withdraw in the box next to the words "Dollar Amount."

    • 5). Place your name on the line next to the word "Name." If there is a space for your address on the withdrawal slip, enter the mailing address associated with your checking account in that space.

    • 6). Enter all the digits in your checking account number in the boxes next to the words "Account Number."

    • 7). Sign your name on the signature line to authorize the transaction.
