- 1). Create a chart using a spreadsheet or word processing document that includes your name, birth date, and blood type.
- 2). Add information about your drug and food allergies, family illnesses and chronic illnesses, if applicable. List any medications that you take along with their side effects and durations of use.
- 3). Write down the name and contact information of your doctors, pharmacist, emergency caregiver and insurance companies.
- 4). Date your last doctor's visits, test results and health screenings. Include a brief description of every procedure.
- 5). Request a copy of any test or procedure reports following your medical visits. Scan the copy into a digital record, or store the file along with your other reports. Store copies of medical bills and insurance claims, as well.
