How to Sign up for CMAPRX

1). Go to the CMAPRX website to sign up for the program. Click on the "Apply" link at the top of the page and click "Patients."
2). Enter your name, address, date of birth, Social Security number, employer and income information. Answer all of the questions asked of you, such as whether or not you smoke.
3). List all of the medications you are currently taking. Sign all of the remaining documents in the presence of a notary public. You must reenter your personal information on all of the pages where lines are provided for your name and Social Security number.
4). Make a copy of your photo identification and send it, along with all signed and completed pages of the application to:

Or you can fax your information to 318-448-4473. You will receive notification from CMAP if your application requires further information, is denied or is approved.
