How to Obtain Free Identification Cards in Pennsylvania
- 1). Complete Form DL-54A, "Application for Initial Photo Identification Card." Fill in all required identification information in Section A.
- 2). In Section B, check option 2, sub-option 1. This is the only free option for an identification card in the state of Pennsylvania.
- 3). Sign and date the application in Section C, then complete the correct information in the Request for Organ Donor Designation section. Sign and date the request.
- 4). Choose two forms of proof of residency. Locate and bring the documents with you. Locate and bring a copy of your birth certificate for proof of your identity. See the application form for acceptable forms of identification.
