The search for pharmacologic agents that provide universally effective prophylaxis against PEP remains elusive, but is ongoing. Although several studies have suggested that NSAIDs are effective in reducing PEP, they have been shown to be of little benefit in higher risk patients. The greatest reductions in the incidence of PEP have been demonstrated through advancements in endoscopic techniques such as pancreatic duct stenting and guidewire cannulation. Contrast injection into the pancreatic duct, an independent risk factor for PEP, can specifically be reduced with guidewire cannulation. Further improvements in endoscopic techniques for DBC will likely yield further reduction in the incidence of PEP.
