How to Fill Out a Death Certificate

    • 1). Fill out section 1 through 23 using permanent black ink. Use information obtained from official legal records, which are more accurate. When filling out the dates, do not abbreviate the months. Do not abbreviate any county names, years or names.

    • 2). Do not fill out sections 24 to 28--this should only be filled out by the pronouncing physician. Sections 29 to 31 should be filled out by the pronouncing physician, certifying physician or medical examiner.

    • 3). Have the certifying physician or medical examiner fill out sections 32 to 45.

    • 4). Ask the person filing the certificate sign to the right of "Signature of certifier." If the person is a doctor, have him list his abbreviated medical degree to the right of his name.
