How to Apply for Ohio Medicaid
- 1). Fill out the Ohio Medicaid application. You will need your social security number, a current address, and contact phone number. You will also need the social security numbers for all members in your household that are applying for Medicaid as well.
- 2). Download and print the Ohio medicaid online application form. Attach a copy of recent check stubs fro proof of income, or an IRS 1040 Tax form (or written income proof from your employer). You must also make a copy of a valid driver's license or state ID. Expecting mothers must accompany an official letter from their doctor on expected due date and number of unborn babies. Provide proof of health insurance copies (front and back) of household individuals with a health insurance plan.
- 3). Make sure that all information is correct and that you have included all required attached documents. Mail the application to your county's Department of Job and Family Services. The county names and addresses are listed below in resources.
