How to Amend a Birth Record in Kentucky

    • 1). Prepare documentation supporting the correction or amendment that needs to be made to the birth record. This could include a Paternity Affidavit or Declaration of Paternity.

    • 2). Call the Kentucky Vital Statistics Office at: (502) 564-4212. They are open from 8 a.m. to 4:30 p.m., Monday through Friday. All amendment requests must be made by phone.

    • 3). Request the change you want made. Supply any supporting documentation that the office asks for--the office will need a copy of your government issued photo identification.

    • 4). Apply for a new birth certificate once the amendment has taken effect.
