    Apply For Mass Health Care

    • 1). Ascertain your general eligibility. If you are not covered by health insurance, and feel you cannot provide it for yourself or your family, you may be eligible for MassHealth Care benefits if you are: an adult whose employer is qualified to provide health insurance, 65 or older years and living at home, a child under the age of 19, disabled, HIV positive, pregnant, an individual of any age who requires long-term care, unemployed and have been so for an extended period, the head of a low income household with children under the age of 19 or 65 or older and caring for relatives under 19 years of age.

    • 2). Obtain an application, also known as a Medical Benefit Request form, from the Mass Health Enrollment Center which you can contact at 1-888-665-9993 or from one of their regional offices located in Revere, Springfield, Taunton and Tewksbury.

    • 3). Obtain an application from the MassHealth website.

    • 4). Obtain the application from a local clinic by asking to speak with a MassHealth benefits advisor.

    • 5). Fill out the application, being sure to answer all questions accurately and honestly. Among answers that will require verification are identity, social security number, citizenship status, residency confirmation, earnings, and any claimed disability. Sign and date the application.

    • 6). Send the application to the MassHealth Enrollment center listed on the application form.
